Connecting Communities to Care 

Connecting Communities to Care (CCC) is HCFA’s ongoing statewide initiative to ensure that low-income residents – especially immigrants, older adults, people with disabilities, families with limited English proficiency, and those experiencing housing instability – can enroll in, renew, and effectively use their health coverage.

From Redetermination Campaign to Statewide Infrastructure 

Connecting Communities to Care (CCC) builds on the success of the Massachusetts Redetermination Campaign – a first-of-its-kind collaboration between Health Care For All (HCFA), MassHealth, the Health Connector, and community partners to help residents maintain health coverage after the COVID-19 public health emergency. When the MassHealth renewal process resumed in April 2023 following a three-year pause, HCFA launched a multilingual, multicultural campaign across 15 high-impact municipalities. Through door-to-door canvassing, community outreach, Certified Application Counselor (CAC) training, and a statewide communications strategy, the campaign reached more than 1.2 million residents and helped thousands successfully renew or transition their coverage.

Now, through CCC, HCFA partners with dozens of community-based organizations across Massachusetts to provide culturally responsive outreach, education, enrollment assistance, and referrals to essential supports such as the MassHealth MyServices portal, Medicare Savings Programs, SNAP, WIC, cash assistance, My Ombudsman, and other critical health and social services. 

Funding Opportunity: FY27 Medicaid Expansion Population – Outreach, Education & Enrollment Grant   

Health Care For All (HCFA) is seeking community-based and faith-based organizations to conduct outreach, education and MassHealth enrollment assistance to help residents maintain their coverage as new Federal Medicaid requirements take effect.  

Many adults enrolled through the Medicaid Expansion program will soon face new requirements to maintain their coverage. They will need to work, volunteer, and/or attend school and report on these activities. They will also be required to renew their MassHealth coverage more frequently. Without clear information and support, many people who remain eligible for MassHealth are at high risk of losing coverage due to paperwork barriers, missed deadlines, reporting confusion, and misinformation.  

Through this funding opportunity, HCFA will support trusted community organizations to provide outreach, education, and enrollment assistance to help residents understand the changes, renew their coverage, and stay connected to health care.
