Advancing Health Equity in Massachusetts (AHEM) is a statewide initiative led by the Executive Office of Health and Human Services (EOHHS) focused on maternal health and social determinants of health.
HCFA’s Role in AHEM
With support from Point32Health and Molina Health, Health Care For All (HCFA) is serving as a backbone partner in AHEM’s five-year initiative focused on maternal health and social determinants of cardiometabolic health in Brockton, Lawrence, and Lowell, including the conditions that contribute to heart disease, stroke, diabetes, and related outcomes.
HCFA’s community engagement role entails working alongside partners to support community-led planning and move shared priorities into action.
