FY27 Medicaid Expansion Population – Outreach, Education & Enrollment Grant

About this Funding Opportunity 

Health Care For All (HCFA) is seeking community-based and faith-based organizations to conduct outreach, education and MassHealth enrollment assistance to help residents maintain their coverage as new Federal Medicaid requirements take effect.  

Many adults enrolled through the Medicaid Expansion program will soon face new requirements to maintain their coverage. They will need to work, volunteer, and/or attend school and report on these activities. They will also be required to renew their MassHealth coverage more frequently. Without clear information and support, many people who remain eligible for MassHealth are at high risk of losing coverage due to paperwork barriers, missed deadlines, reporting confusion, and misinformation.  

Through this funding opportunity, HCFA will support trusted community organizations to provide outreach, education, and enrollment assistance to help residents understand the changes, renew their coverage, and stay connected to health care. As federal guidance continues to evolve, outreach and education activities will also incorporate additional public health priorities, such as vaccine outreach, in coordination with MassHealth and the Massachusetts Department of Public Health. Organizations will also help community members connect to other important supports, such as SNAP, WIC, the Medicare Savings Program, and the MassHealth My Services portal. These supports help families maintain stability and access the care they need. 

Health Care For All (HCFA) leads Connecting Communities to Care (CCC), a statewide network of community-based and faith-based organizations providing culturally responsive outreach, education, and MassHealth enrollment assistance. Originally launched through state appropriations in partnership with MassHealth and the Health Connector to support residents during post-pandemic redeterminations, CCC has helped thousands maintain health coverage through trusted, community-based engagement. 

The Massachusetts Legislature appropriated funding for HCFA to lead a statewide campaign to address upcoming federal Medicaid changes. As part of this effort, HCFA is expanding the CCC initiative and recruiting additional community organizations to focus specifically on the Medicaid Expansion population in Massachusetts – adults ages 19–64 without dependents or qualifying disabilities – who will face work requirements and twice-yearly eligibility redeterminations beginning in January 2027. 

 

Funding Structure (FY27 – One-Year Awards with possibility of renewal) 

Organizations may apply for one of two funding levels: 

Outreach & Education (O&E) Only – $40,000 (FY27)
Organizations will conduct culturally and linguistically responsive community education, awareness-building, and referrals related to MassHealth changes and renewal requirements. Outreach and education activities will also incorporate vaccine outreach, and referrals to supports, such as SNAP, WIC, the Medicare Savings Program, and the MassHealth My Services portal. These supports help families maintain stability and access the care they need. 

Outreach & Education + Certified Application Counselor (CAC) Services – $70,000 (FY27)
Organizations will conduct outreach and education as described above and provide direct MassHealth enrollment assistance, including applications, renewals, redeterminations, and ongoing eligibility support. Applicants for this level must be an existing CAC site or commit to completing CAC training through HCFA. Organizations must commit to having two CACs that will support organizational office hours and enrollment events.  

Geographic Awards 

HCFA will award additional funds to organizations based in the following communities: 

  • Boston (Dorchester) – O&E + CAC 
  • Chicopee – O&E + CAC 
  • Fall River – O&E + CAC 
  • Haverhill – O&E + CAC 
  • Lawrence – O&E only (2 award) 
  • Lowell – O&E only (1 award) 
  • Lowell – O&E + CAC (1 award) 
  • New Bedford – O&E only 
  • Revere/Malden – O&E + CAC 
  • Springfield – O&E + CAC 
  • Somerville – O&E only 
  • Taunton – O&E only 
  • Western Massachusetts (regional) – O&E + CAC 
  • Worcester – O&E + CAC 

These additions will complement the existing CCC network, which will continue serving other priority populations, including the people facing homelessness statewide. Selected partners will join a coordinated, statewide campaign led by HCFA and MassHealth, receive training and technical assistance, and participate in shared messaging, reporting, and evaluation efforts. 

The FY27 grant period will begin July 1, 2026, and run through June 30, 2027, with possibility of renewal. 

Funding Timeline 

The FY27 expansion grants are one-year awards supporting work from July 1, 2026 through June 30, 2027 with the possibility of renewal. 

Applications will open on March 23, 2026, and close on May 8, 2026. Finalists will be notified by May 15, 2026, allowing selected organizations time to prepare for a July 1 start date. 

Who Should Apply 

HCFA welcomes applications from community-based organizations and faith-based organizations that serve adults enrolled through the Medicaid Expansion category (ages 19–64) or communities at high risk of coverage loss (e.g. people facing homelessness). Applicants must be physically located in, and primarily serve, the specific city or town for which the award is designated.  

We are particularly interested in organizations with strong community trust, deep local relationships, and lived experience connections to the populations they serve. Organizations do not need to be large to apply; smaller, trusted groups are strongly encouraged. Please note: Community Health Centers are not eligible for this grant.

What We Are Looking For 

HCFA seeks partners that can reach residents at risk of losing coverage and provide linguistically and culturally responsive engagement. Organizations should be prepared to deliver consistent outreach, education and enrollment support – not one-time events – and to participate in coordinated statewide efforts as part of the CCC network. 

Selected partners will receive training and technical assistance, shared messaging guidance informed by statewide research, enrollment support resources, and ongoing coordination from HCFA to ensure alignment and impact. 

Application Process 

The application will be a short online form. Applicants will be asked to provide a brief overview of their organization, the target population and geography they propose to serve, a description of planned activities, estimated reach or number of individuals assisted, a staffing plan, and a brief budget. Formal proposals and lengthy attachments will not be required.
